What you are paying for

A check-up is an examination, a soft-tissue check for anything abnormal, and an assessment of gum health. Radiographs are often included for a new patient and charged separately afterwards. Ask which, because a £45 check-up with £30 of X-rays is not cheaper than a £75 one that includes them.

How often you actually need one

NICE guideline CG19 does not recommend six-monthly check-ups for everyone. It recommends an interval set individually between 3 and 24 months, based on your risk of dental disease, and its 2018 surveillance review found no evidence that a 24-month interval harmed low-risk adults. If you are recalled every six months by default, it is reasonable to ask what your risk assessment was — over ten years the difference is substantial.

The NHS alternative

An NHS check-up is a Band 1 charge of £27.90, which covers the examination, any X-rays and a scale and polish if you clinically need one. That price is set nationally and does not change with the postcode. The barrier is availability, not cost.

What the evidence says

A routine scale and polish every six months does not keep gums healthier than having one when your dentist judges it necessary.

The Cochrane review found that scheduled scale and polish did not reduce the early signs of gum disease more than treatment given as needed. It does remove more calculus — six-monthly more than twelve-monthly, on high-certainty evidence — but the clinical importance of that difference is uncertain. Patients did report their teeth felt cleaner. If a practice sells you a fixed cleaning schedule, this is the evidence to ask them about.
